If submitting a form electronically, it is highly recommended to submit the form through the UH West O’ahu Secure File Transfer. The suggested file format is PDF. If you don’t have access to a scanner, you can scan documents using your smart phone.

Authorization to Pick Up Official Documents (PDF) | Designate a third party to pick up official documents. |
Change Grading Option | Request to change grading option to CR/NC or Audit. |
Consent to Disclose Educational Records to Third Party | Request to disclose educational information. |
Course Substitution | Request for a course substitution. |
Directed Reading and Research Course Registration | Request to enroll in a Directed Reading Research class. |
Enrollment Verification Request | Request for enrollment verification. |
Health Clearance Requirements | Provide proof of DOH Health Requirements and upload to Med+Proctor. |
Late Registration Request | Request to add a course after the add period ended. |
Non-Disclosure of Directory Information | Request to not disclose directory information. |
Petition for Back Credits for Languages | Petition to earn Language Back credits. |
Petition for Leave of Absence | Petition for a one semester leave of absence. |
Petition to Graduate in Absentia | Petition to graduate in absentia. |
Registration and Records Student Appeal | Appeal a course drop/withdrawal or academic standing. |
Replacement Diploma | Order a replacement diploma. |
Revocation of Consent to Disclose Educational Records to Third Party | Request to revoke consent to disclose educational information. |
Senior Citizen Visitor Program Application (PDF) | Request to participate in regular credit classes. |
Student Data Change | Request to change name / preferred name, update address or update phone number. |
Transcript Forwarding Request | Request to forward copies of non-UH transcripts to another UH campus. |
Transcript Request | Request for official transcript. |
Veteran Student Change in Enrollment | Request to update certification upon making changes to courses. |
Veteran Student Concurrent Enrollment | Request to certify concurrent enrollment at other UH campuses. |
Veteran Student Enrollment Certification | Request to certify UH West Oʻahu courses. |
