Steps to Complete a Request for an ID Card
Step 1. Confirm Payment of Tuition
Check your student account to confirm all tuition and fees are paid in full. Students can sign up for the UH Payment Plan. Students receiving financial aid, Veterans Affairs benefits, or tuition assistance from a third party should contact those offices to confirm if payment was received.
Step 2. Complete the University ID Card Application
Complete and submit the online University ID Card Application with the required items listed below.
Step 3. Submit a copy of a valid photo ID
You will need a copy of a valid color photo ID (student photo ID, driver’s license, state ID, or government issued ID). Please be sure copy is clear and readable. Screen shots are allowed.
Step 4. Submit a Current Photo
You will need a current photo which will be used for your new UH West Oʻahu ID card. Please see sample below. The photo must have a neutral background and good lighting. No sunglasses or hats. Hair must be cleared from face.

Step 5. Submit Required Items
Complete and submit an online University ID Card Application. You will be able to attach a copy of your valid photo ID and your current photo. All items will be submitted online securely.
Your UH ID card will be mailed to you. Please confirm that your mailing address and contact phone number are current by checking your MyUH account.
- Go to: MyUH Services
- Select: Student Services Menu
- Select: Personal Information tab. “View/Update” mailing address and “View/Update” phone number.
