If submitting a PDF form electronically, it is highly recommended to submit the form through the UH West O’ahu Secure File Transfer at https://westoahu.hawaii.edu/it/securefiletransfer. If you don’t have access to a scanner, you can scan documents using your smart phone. All of the following forms are stored in Adobe Portable Document Format (PDF). To view and print PDF documents, you must have Adobe Reader installed on your computer.
