How do I add tagged form fields in Adobe Acrobat?

For Adobe Acrobat Pro DC

1. Click on Tools > Accessibility > Full Check.
2. Click on the tags icon on the left hand side.
3. Right-click “Tags” and select Find…
   
4. Select “Unmarked annotations” in the drop-down box.
5. Select “Search Document” radio button and click Find.
   
6. If an untagged annotation is found, you will see a message appear with the format “Type: [Tooltip message]”
7. Click Tag Element.
   
8. Click “Form” in the drop down box.
9. Provide a Title.
10. Click OK.
    
11. Repeat steps 7 – 10 until no more untagged annotations are found.
