Instructions
  1. Submit this proctor form and necessary exam materials at least 10 business days prior to the start of the testing window.
  2. Instruct student(s) to plan ahead. Test Appointments are scheduled (72) hours in advance at registerblast.com/uhwestoahu. Students must bring a valid photo ID to their test appointment.
  3. Discuss exam guidelines (i.e approved test materials, test date range, and test times) with your students.
Policies and procedures
  1. All exam materials and guidelines must be received directly from the instructor.
  2. Remote Proctor Services are only available for online computer based exams.
  3. In order to ensure test integrity, changes, minor corrections, or revisions to exams will NOT be made by the Noʻeau Testing Staff. A revised test must be submitted by the instructor.
  4. If exam materials are not received (24) hours prior to a studentsʻs exam appointment, the exam appointment will be rescheduled to a later date.
  5. Students are asked to adhere to the Pueo Safety Promise and Safe Return to Campus Guidelines.
  6. Students who prefer on-campus exams will be asked to complete a voluntary temperature check.
