EUTF Open Enrollment 2025 Confirmation Notices
A message from UH West Oʻahu Human Resources and the Hawaiʻi Employer-Union Health Benefits Trust Fund to EUTF-eligible employees in an email dated June 18:
EUTF Open Enrollment (OE) 2025 Confirmation Notices (CN) are now available in the EUTF Member Portal!
All employees will have a 2025 OE CN, which will be available in the Member Portal. Hard copy CNs will not be mailed.
If you made changes to your enrollment during OE and submitted all required documents timely, your CN will reflect these changes.
If you did not make changes during OE, you were automatically defaulted into your current coverage, which will be reflected on your CN.
You can access the EUTF Member Self-Service Portal from a desktop, smart phone, or tablet at https://eutfbenefits.hawaii.gov/.
Please reference the Member Self-Service Portal Guide for detailed instructions on registering for the first time.
Viewing your Confirmation Notice is quick and easy! Simply follow these steps after logging in:
1. Click on Communication Center located in the black navigation bar at the top, right of the home page.
2. Click on the ConfirmationStatementActiveAE.pdf hyperlink to download your Confirmation Notice.
3. Check your download folder for a PDF of your Open Enrollment Confirmation Notice.
Please be advised that all Open Enrollment Changes are final.

Identification Cards
If you and/or your dependent(s) are newly enrolling in a plan option, you will receive identification cards in late June.
ID cards are not issued for Verdegard Administrators (supplemental medical and prescription drug), American Specialty Health Group, Inc. (chiropractic coverage included with the medical), VSP (vision plan), and Securian Financial (life insurance), as ID cards are not required for these services.
Please contact the carriers directly to follow up on the status of your ID card if not received by early July.
