Financial Aid Forms
The Financial Aid Office may request additional forms and documents from you in order to complete your financial aid application. If you are required to submit anything additional to our office, you can view these requirements on the ‘Financial Aid’ tab on your MyUH Student Portal.
To view and print PDF documents, you must have Adobe Reader installed on your computer. If there is an eForm that you wish to have available as a PDF, please contact the Financial Aid Office.
If submitting a PDF form electronically, it is highly recommended to submit the form through the UH West O’ahu Secure File Transfer.
